WHEREAS, the City of West Memphis has contracted with
Prudential Insurance Company of America for hospital benefits.
~~ for the City Employees for the year, 1881; and,
'fir WHEREAS, the City of West Memphis wishes to formally
adopt the Prudential Insurance Company of America's plan for
hospital benefits for the City Employees for the year, 1981;
THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF WEST MEMPHIS, ARKANSAS;
1. That the City of West Memphis formally adopt the
plan of group health and life insurance submitted by Prudential
Insurance Company of America. A brief description of the plan
of coverage is attached.
2. That the City of ~niest Memphis shall be responsible for
the Employees' share of premiums;
3. That the Employees be responsible for paying prerniurgs
for benefits available for dependent coverage;
4. That Blue Cross-Blue Shield shall be notified by
Certified Mail, Return Receipt Requested in cancelling the
contract made with them b,y the City of West Memphis, Arkansas.
